Semaglutide, marketed under the brand names Wegovy and Rybelsus, is a prescribed medication used to manage type 2 diabetes and assist with weight management in individuals with obesity. Wegovy and Rybelsus for weight loss, what is the difference?

All semaglutide formulations used in weight loss medication are GLP-1 receptor agonists, which suppress the appetite by slowing the rate at which food moves from the stomach into the small intestine, making you feel full faster and longer. 

Wegovy and Rybelsus work the same in the body, but some differences exist. Rybelsus is an oral (taken by mouth) pill that is FDA-approved to treat diabetes.1 Wegovy is an injectable medication; it is the first glucagon-like peptide-1 (GLP-1) receptor agonist approved for long-term weight management. 

The Difference in Semaglutide Medications

The differences between the various semaglutide weight loss products can be confusing. Here are the primary differences:

  • Wegovy and Ozempic are semaglutide injections; Rybelsus is a tablet. 
  • Wegovy and Ozempic are taken once weekly; Rybelsus is taken daily. 
  • Wegovy is FDA-approved for chronic weight management; Rybelsus and Ozempic are approved to treat type 2 diabetes.
  • Wegovy comes in a higher dose (2.4 mg) than Ozempic (0.25-1mg). Rybelsus dosage varies from 3mg to 14 mg.

Like all forms of semaglutide, Wegovy and Rybelsus suppress the appetite, which results in weight loss. All forms of semaglutide also stimulate the pancreas to secrete insulin, which lowers blood sugar. Wegovy and Rybelsus are glucagon-like peptide 1 (GLP-1) agonists. GLP-1 is a hormone that controls appetite and hunger by binding to brain receptors.

Ozempic and Wegovy are injectable forms of semaglutide, but they are not the same. Because Ozempic was shown to cause weight loss in clinical trials, researchers began testing high-dose forms of semaglutide to treat obesity. Wegovy, an injectable form of semaglutide with a higher maximum dose (2.4mg) than Ozempic, was developed specifically to treat obesity. Wegovy was FDA-approved in June 2021.

In the Wegovy clinical trials, adults with obesity or who were overweight with a weight-related medical condition received either Wegovy or placebo, along with a reduced-calorie diet and exercise plan. After 68 weeks, study participants taking Wegovy had an average weight loss of 14.9%, compared with 2.4% in the placebo group.2

Rybelsus weight loss

Rybelsus is an oral pill taken daily that is approved to treat type 2 diabetes in adults. Although the FDA hasn’t approved it for weight loss, doctors often prescribe Rybelsus as an off-label drug (i.e., one that is given for a condition other than FDA-approved use) to help with weight control. Higher doses of Rybelsus are being studied for the drug’s effect on weight loss. Initial results suggest it may result in an average of 15% body weight loss.3

Of 196 reviews on Drugs.com, nearly half of Rybelsus reviewers reported a positive experience.4  It’s important to note that most of the reviewers took Rybelsus for type 2 diabetes, and many were on several antidiabetic medications along with Rybelsus. Here are a few reviews from people who take Rybelsus for weight loss: 

1. Soso – Reports initially feeling nauseated when taking Rybelsus, but it dissipated by the third day of taking the medication. By day three, Soso said, “I could tell I wasn’t as hungry as before. I don’t look for snacks, I don’t overeat my dinner, and I get full faster.” Soso reports losing 25 lbs. after taking the medication for under two months.

2. Pauline: “I have lost 1 stone [14 lbs.] exactly after 7 weeks on Rybelsus. I have next to no symptoms, occasional heartburn, and a sudden dislike for my morning coffee.” Pauline says she’s satisfied eating under 1400 calories daily and has a BMI of 25 for the first time in 10 years. She also mentions losing her food obsession and rarely feeling hungry.

3. Tom—weighed 345 lbs. He reports, “Doc made it clear my blood work was unacceptable for a 42-year-old (A1C was 6.4, fatty liver diagnosis, and borderline cholesterol).” After two months, Tom states his A1C is down to 5.5, he lost 25 lbs., and his cholesterol and liver enzymes are back to normal.

In a randomized, double-blind, placebo-controlled clinical trial, the gold standard of medical studies, 667 people with obesity were given either a 50 mg Rybelsus oral (by mouth) tablet or a placebo (sugar pill) each day for 68 weeks. The average weight loss at the end of the study was 15.1% for the group that took oral semaglutide (Rybelsus) 50 mg daily. The placebo group lost only 2.4% of their body weight.5

The study authors wrote, “In adults with overweight or obesity without type 2 diabetes, oral semaglutide 50 mg once per day led to a superior and clinically meaningful decrease in body weight compared with placebo.” 5

Wegovy reportedly has an average rating of around 7.2 out of 10 from 312 online reviews for obesity treatment.6 Samples of real-life scenarios from Wegovy users include:6

Mike—Mike had been taking Wegovy for four months at the time of the review; Mike stated that the “drug has been life-changing.” Mike was 357 lbs. when he started taking Wegovy 2.4 mg, and at the four-month mark, he reported having lost 60 lbs. Mike reported some minor gastrointestinal side effects (heartburn and stomach upset) early on, which have since subsided. 
  2. 2. Kelly—”I have been on Wegovy for almost seven weeks and have lost over 22 lbs.,” Kelly reports. She says her weight loss progress is steady without dieting or exercising. Kelly denies having experienced any “bad side effects” from taking Wegovy. “From the moment I took the first injection, the food noise was gone,” she says. 
  3. Anonymous- “I have been on Wegovy for over a month now; it has been a life changer for me.” This anonymous reviewer of Wegovy says they have lost nearly 14 lbs. since starting on Wegovy, limiting eating hours to 6:00 a.m. and 3:00 p.m. and walking 1/2 mile daily during lunch breaks.
